A P P O I N T M E N T S

We offer several ways for you to book an appointment. You may book your appointment online through our website, www.modernholisticskin.com, by email at hello@modernholisticskin.com, or by calling/texting us at 404.808.6519. A credit card is requested to secure and guarantee all appointments. We strictly enforce a 24-hour cancelation and rescheduling policy. It is recommended that you schedule your holistic experience with as much advanced notice as possible to ensure your preferred date/time, or we strongly suggest rebooking an appointment after checkout. All holistic experiences are subject to availability. As a courtesy, you will receive a 72-hour email reminder and a 24-hour text message appointment reminder. Menu service and prices are subject to change without notice. We reserve the right to refuse service to anyone. *See below for payment information.

N E W C L I E N T  

All new clients must book a New Client Experience, either facial or massage therapy. We will not see a new client without consultation. If the available appointment time frame does not allow for both consultation and facial/massage experience, you will need to select an alternate date/time. Our 'signature' NEW CLIENT EXPERIENCE does include both consultation and service time; therefore, the selection of a consultation is not needed.  

Please arrive 10-15 minutes before your starting appointment time to give yourself ample time to start your relaxation in our Grounding Lounge.

A 15min consultation is complimentary for the first appointment.

A R R I V A L

We kindly ask that you sanitize your hands upon arrival with our provided organic hand sanitizer at our front entrance. Please remove your shoes and replace them with provided spa slippers upon arrival. This ensures our spa environment is healthy and clean for all employees and clients. We offer signature spa slippers, a warm neck pillow, and beverages and nuts for your comfort in our Grounding Lounge. Delayed arrival will limit the time for your experience, reducing the effectiveness of your therapy and the expectations of your visit. In consideration of other scheduled clients, service time will not be extended for delayed arrivals, and the full price of your service will be charged. Please notify our staff if there are any special physical or medical needs or conditions they need to consider prior to your services (i.e., pregnancy, food, or product sensitivities or aggressive medications). We recommend that valuable items be left at home. MODERN Holistic Skin is not responsible for lost or stolen items.

C O N S U L T A T I O N + W A I V E R

All New Clients must complete a consultation form, as well as the consent of policy and procedures waiver. MODERN Holistic Skin strives to uphold a “judgment-free” environment. We respectfully ask that you kindly provide as much detailed information as possible in your consultation form. Consent to provide health and wellness information to the best of your ability and knowledge and agree to inform MODERN Holistic Skin of any changes. This is for your safety. This includes past and current health diagnoses, conditions, disorders, and diseases; all current medical prescriptions, supplements, including those prescribed by a Psychologist, General Practitioner, Dermatologist, as well as any aesthetic medical produces and/or injectables. If you have any injectables within 4-weeks/MONTH of your scheduled appointment, please reschedule to allow ample time for healing. If you have had Botox administered within 4-weeks/MONTH of a scheduled appointment, you must rebook to allow ample time for healing. If you are currently using any Retin-A products, over-the-counter, or by Rx, please discontinue use for 5-7 DAYS prior your scheduled appointment. If you are scheduled for a waxing appointment and these measures have not been taken and the service can not be performed, you are subject for full charge of waxing service. I have been informed of and understand the contraindications to the requested treatment and agree that I do not have any condition(s) that would make the requested treatment unsuitable. I will inform the Skin Therapist/Massage Therapist of any discomfort I may experience at any time during my treatment to allow them to adjust accordingly. I agree to waive all liabilities towards employer, Skin + Spirit LLC, for any injury or damages incurred due to any misrepresentation of my health history.

24- HOUR | R E S C H E D U L I N G + C A N C E L L A T I O N P O L I C Y

As a courtesy, you will receive an email confirmation of your appointment upon booking as well as a 72-hour reminder email and a 24-hour reminder text message. A no less than a 24-hour notice is REQUIRED to either RESCHEDULE OR CANCEL YOUR APPOINTMENT. Insufficient notice to reschedule or cancel appointments is subject to charges up to full-service value. Should the need arise to cancel within 24-hours of your starting appointment time, your credit card on file will be charged for the full-service amount. Should the need arise to reschedule your appointment within 24-hours of your starting appointment time, you must reschedule your appointment within three days, service schedule availability pending, to avoid full-service charges. Any late rescheduled appointments may not be canceled or rescheduled at a later date. *If the need arises to cancel/reschedule a second time, this will ensure full cost of service, not avoid it. If an appointment cannot be rescheduled within three days, the client will be charged for the full-service amount using the credit card on file. 48-hour advance notice is required for any spa packages, which includes multiple holistic therapies lasting more than 2 hours. All NO SHOW appointments will be assessed a charge at full value of services. Spa and Bridal Parties are subject to terms of agreement for cancellations.

S P A R E Q U I R E M E N T S

We accept clients starting at age 13, however, services may require parental release before the services can be rendered. All minors must be accompanied by an adult. To ensure that all guests may experience a pleasant and peaceful environment, we ask that only those receiving services accompany you to the spa.

P A Y M E N T + G R A T U I T I E S

To secure your appointment, a credit card is required at the time of booking.
All personal + financial information is kept strictly confidential.

We accept credit cards and gift certificates only.

Gratuities may be charged to credit card or accepted in cash, although we cannot provide change. There will be a $50.00 fee each time a check is returned as well as any incurred charges for collection of payment owed.

Gratuities are not included in the value of the services and are at the discretion of the client. 

G I F T C E R T I F I C A T E

MODERN Holistic Skin gift certificate contains value which may be used to purchase holistic services and products at full price value. This gift certificate cannot be used to purchase other gift cards. The gift certificate number must be presented at time of purchase and the available balance will be applied to the transaction. Any remaining balance shall remain on the card for future use until balance is expired. Prices are subject to change without notice. Redemption of this card is subject to certain restrictions and expirations set by each spa location. Should this card be redeemed at another location, the card will be redeemed at face value. This card is non-refundable and may not be redeemed for cash. Gift certificates expire 365 days after date of purchase.

This card is not transferable and MODERN Holistic Skin is not responsible for any lost, stolen, damaged or unauthorized use of the card.

P R O D U C T S + R E T U R N S

In keeping with MODERN Holistic Skin, purity and quality are essential elements in our skin and body care products. In the event you are not satisfied with a skin or body care product, you will be given spa credit if the product is returned within 30 days of purchase. For other retail items, a refund will be issued as spa credit or back in the form of which payment was received within 30 days of purchase.

S E R I E S + P A C K A G E S

All series packages have an expiration date of 90-days. This is to ensure you receive the desired results. Series are weekly standing/scheduled appointments until expiration of purchased series. If the number of appointments within a purchased series is not completed within expiration date, any future booked appointment is will be charged a full value. A refund of outstanding appointments will not be applied.

R E S P E C T + C O N S I D E R A T I O N

All payment and personal health and wellness information is strictly confidential, unless authorized. No discussion of politics is allowed between client and therapist at any time. Our establishment will not be affiliated with any political party and our environment is not the place for such discussion. This will not be tolerated at any point in time.
